Can AI Recognize a Medical Emergency? New Study Raises Serious Concerns

Artificial intelligence is increasingly being used for health information. Millions of people turn to AI chatbots every day to ask medical questions — from minor symptoms to potentially life-threatening concerns.

But a new study published in Nature Medicine raises an important warning: AI may not reliably recognize when someone needs urgent medical care.

At Orleans Family Health Clinic (OFHC), we believe patients deserve clarity, safety, and medically sound guidance — especially in urgent situations.

Let’s break down what this study found and what it means for you.

What the Study Examined

Researchers led by Dr. Ashwin Ramaswamy at the Icahn School of Medicine at Mount Sinai evaluated the “Health” feature of OpenAI’s ChatGPT platform.

They created 60 realistic patient scenarios ranging from:

  • Mild illnesses
  • Worsening chronic conditions
  • Clear medical emergencies

Three independent physicians determined the appropriate level of care for each case. The researchers then tested how ChatGPT Health responded under different variations of those cases.

The Most Concerning Finding: Under-Triaging Emergencies

In 51.6% of cases where hospital care was urgently required, the AI advised patients to:

  • Stay home
  • Wait
  • Book a routine appointment

In certain asthma scenarios, early signs of respiratory failure were identified — yet emergency care was not recommended.

The platform also:

  • Over-referred safe individuals to emergency care in 64.8% of non-urgent cases
  • Downplayed symptoms nearly 12 times more often when a “friend” in the scenario suggested it wasn’t serious

In healthcare, triage accuracy is critical. Missing a true emergency can have devastating consequences.

A Particularly Serious Issue: Suicide Ideation

The researchers also tested responses to suicidal ideation.

When a patient stated he had been thinking about taking a large number of pills, the system initially displayed crisis resources.

However, when normal lab results were added to the same scenario:

  • The crisis intervention banner disappeared in every test attempt.

That inconsistency is deeply concerning.

At OFHC, we take mental health symptoms seriously — and suicidal thoughts always require immediate professional assessment.

If you or someone you know is in crisis in Canada:

  • Call or text 9-8-8 (Suicide Crisis Helpline)
  • Call 9-1-1 for immediate danger
  • Go to the nearest emergency department

Why AI Tools Can Be Problematic in Medical Triage

AI systems:

  • Do not perform physical exams
  • Cannot assess subtle clinical cues
  • Do not assume medico-legal responsibility
  • May weigh contextual information inconsistently

Even when they perform well in “textbook” emergencies (e.g., stroke or anaphylaxis), real-world presentations are rarely textbook.

Medical decision-making requires:

  • Clinical training
  • Experience
  • Contextual judgment
  • Risk stratification

These are human competencies developed over years of practice.

What This Means for Patients

AI can be helpful for:

  • General education
  • Understanding terminology
    Preparing questions for your doctor

AI should not replace:

  • Emergency medical assessment
  • Urgent triage decisions
  • Mental health crisis intervention

If you experience symptoms such as:

  • Chest pain
  • Difficulty breathing
  • Sudden weakness or confusion
  • Severe abdominal pain
  • Signs of stroke
  • Thoughts of self-harm

Seek urgent medical care immediately.
